policy. Our Billing
Specialists are always willing to discuss our professional fees
and any needs and/or concerns you may have regarding them.
How May I Pay?
We accept payment by personal check, cash,
Visa, MasterCard, American Express or Discover.
Do I need a Referral?
If you have an HMO plan, with which we are
contracted, you will need a referral from you primary care
physician. It is your responsibility to obtain a valid referral, prior
to being seen. If
you are unable to obtain the referral by the time of your visit,
you will be rescheduled. Please
contact our billing department to verify if we are participating
with your HMO plan.
Which Plans Do We Contract With?
The basic companies are Medicare,
Blue Cross Blue Shield of Florida, Health Options, AvMed, and
Cigna. Please contact our billing department to verify if we are
participating with your company or network not listed here.
Surgery
If your physician recommends surgery, we will
complete any certification/authorizations needed by your insurance
company.
We will bill your insurance company as a
courtesy to you. Depending
on your insurance coverage, you may have charges not covered by
your insurance company, as well as applicable co-payment and
deductibles.
We do employ physician assistants to provide
medical care prior to and during surgery.
We do bill for their services, however, it may or may not
be a covered service with your insurance company.
We advise you consult your insurer to see if this is a
covered benefit.
What is My Financial Responsibility for
Services?
It is imperative we have the correct
information from you regarding your plan and coverage.
The agreement of the insurance company to pay for your
medical care is a contract between you and your insurance company.
Please understand you are ultimately responsible for
payment of your bill. You
may have responsibility for copays at the time for service,
coinsurance, deductibles, and balances not paid by your insurance
company. Again, if
you have any questions or concerns, please feel free to contact
our Billing Specialist.
Our Pledge
Our pledge to you, our patients, is that we
will always place your best interest first, regardless of your
health coverage. Your
medical treatment will not be selected to maximize our
reimbursement, nor will your treatment be withheld for our
financial benefit. We
do believe in assisting our patients with understanding their
financial responsibilities, and will attempt to be sensitive to
financial issues that affect you.
